Student Forms

Important University forms are on the Registrar's site. These include the Change of Major, GE Substitution, and Graduation Application forms (and many more).

If you have agreed with your advisor on a course substitution for a course in your major, your advisor must file that form. It is linked from the Faculty Forms page.
